2. Download the Attached W-9 Form and Complete
Download and complete a W-9 form. After completing the W-9, please email it to support@onehearthealingcenter.com or mail a copy to the physical address below.

3. Receive Monthly Referral Earnings
You will receive a payout every month you earn at least $50 in referrals. We will process payments on the 28th of each month for the previous month’s sales.
